Please print this application form, answer the questions, then fax it with your resume (if you have one) to 816-222-7879. You will be contacted within 24 hours to schedule a pre-qualifying coaching consultation.

All career coaching groups are formed on a first come - first enrolled basis. Groups must have at least 4 participants to start. No more than 8 participants will be enrolled in any one group.

A coaching service agreement will be emailed to you following our phone consultation. You will need to complete and fax back the service agreement with payment information before being officially accepted into any group coaching program. All payments are final. No refunds after payment.
